Cancellation Policy

APPOINTMENT CANCELLATION POLICY

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in your therapist's day that could have been filled by another patient.

When we schedule your Rehab, Strength Training, Male Pelvic Floor PT or Therapeutic Laser Treatments, our system will send timely reminders, updates and notifications about your appointment with ample time to cancel or make changes if needed. We require a 48 hrs notice of any PT session cancellations to avoid late cancellation fees.

Patients who provide less than 48 hours notice up to 8 hours prior to their appointment will be charged $150 late cancellation fee to the card on file.

Patients who notify us 8 hours or less prior to their appointment, or miss/no-show their appointment altogether will be charged a full session fee:

Cancellation Fees are as follows: Evaluations: $285 Subsequent Visits: $265 Laser $75

Please refer to your intake forms for details about late cancellation fees or reach out to us directly. Failure to sign Fiziologix’s intake form before your PT appointment does not exempts you from this policy. For best and fastest results, it is important that you attend all of your booked sessions. We are committed to your recovery.

As courtesy to our clients, if your appointment spot gets released and re-booked online by another client, we may make an exception to this policy for first time late cancellations or documented emergencies. No exceptions are made for last minute cancellations or no-shows.
