Health care coverage for uniformed service members, retirees, and their families.
We accept Washington State Department of Labor & Industries claims for workplace injuries covered under workers' compensation.

We are committed to maintaining privacy and confidentiality on behalf of all of our patients including medical records as well as any information exchanged between us and your insurance provider, doctors or hospitals.

Call us directly and we'll help you sort it out. We verify in-network benefits through Availity as a courtesy — though we always recommend confirming the details of your PT benefits directly with your insurer before your first visit.
Billing note:
Co-pays are due at the time of service. Co-insurance is due at the time of claim payment. Both are charged to the card on file. A 10% late fee applies to outstanding balances past 30 days due to expired or declined cards. A valid card on file is required to continue treatment.
Out-Of-Network Plans
FOR SELF-PAY + SUPERBILL
We work with For PPO plans not listed above — including Aetna, United Healthcare, Molina, Provider One, Ambetter, and most Medicare Advantage plans — we are an out-of-network provider. Many PPO plans still carry out-of-network PT benefits, and if you've met your annual out-of-pocket maximum, your plan may cover 100% of visit costs.
In these cases, you pay at the time of service and we provide a Superbill — a detailed receipt you submit directly to your insurance for partial or full reimbursement, depending on your plan's out-of-network deductible.
HMO or EPO plan? These plans typically do not include any out-of-network coverage. Your option would be to self-pay for services. Contact us and we'll walk you through what that looks like.
Please note we are phasing out our Medicare contract. We will not be accepting Medicare patients and are legally barred from billing directly except for Class IV laser treatments.
PERSONAL INJURY PROTECTION (PIP) AUTO INSURANCE
In Washington state, Personal Injury Protection (PIP) auto insurance covers medical expenses like physical therapy with no out-of-pocket costs, regardless of fault. Use your auto accident claim number to book appointments with our clinic and we will handle all of the paperwork with your auto insurance, attorney or case manager at no additional cost.

We get this question, and we take it seriously. The honest answer is that some insurance models restrict how we treat — limiting visit types, frequency, and approaches in ways that don't always align with what's best for you. Staying out-of-network with certain plans gives us the freedom to deliver care that's truly personalized, evidence-based, and built around your recovery — not a coverage checklist.
We're always evaluating our network relationships and working to expand access. In the meantime, we're committed to making our services as accessible as possible through Superbills, transparent pricing, and flexible scheduling. If cost or coverage is a concern, talk to us — we'll work with you to find the best path forward.
