In-Network Insurance Providers

Our physical therapy clinic on Bainbridge Island is currently in-network/contracted with some of the biggest insurance providers: Tricare, Medicare, Regence Blue Shield, First Choice Health Network, Washington State Department of Labor & Industries.

Please call us directly if you don’t see your insurance provider on this page. This list keeps growing and many companies are affiliated or are what’s called a “secondary” insurance. We’d be happy to review your benefits with you and recommend best options for care. We also accept cash payments which gives us 100% flexibility to customize treatment plans specifically for you, to get you feeling better, faster.

Medicare Part B

Medicare is funded through the Hospital Insurance Trust Fund and the Supplementary Medical Insurance Trust Fund.

Please note: UnitedHealthcare offers a Medicare Advantage Program which is not the same as Medicare Part B with UnitedHealthcare as your secondary. We are not contracted with UnitedHealthcare and do not work with them on an out-of-network basis.


First Choice Health Network

First Choice Health, headquartered in Seattle, WA, is a provider-owned healthcare organization offering employers a forward-thinking alternative to traditional health insurance. Regence Blue Shield of Washington

With roots tracing back to the first BlueShield plan in the United States, Regence is a not-for-profit organization and the largest affiliation of health care Plans in the Pacific Northwest/Mountain region.


Tricare

TRICARE is the health care program for uniformed service members, retirees, and their families around the world.


Fiziologix Physical Therapy services are available for those who file a claim through their employer’s workman’s compensation insurance.


Fiziologix Physical Therapy Clinic is HIPAA compliant.

We are committed to maintaining privacy and confidentiality on behalf of all of our patients including medical records as well as any information exchanged between us and your insurance provider, doctors or hospitals.

If your plan has a copay for specialists, please bring cash, check or credit card to your PT appointments. Upon establishing a treatment schedule with your PT, your appointment attendance is crucial to the effectiveness and speed of your recovery. The schedule also ensures strength building over time to prevent injury. We encourage you to commit to your wellbeing and avoid cancelling your appointments unless absolutely necessary.

Out-Of-Network Insurance Providers

For all other PPO insurance plans (such as Premera Blue Cross, United Healthcare…), FizioLogix Physical Therapy is an out-of-network provider. Many PPO plans have good out-of-network coverage, and if you’ve met your annual out-of-pocket maximum they typically pay 100% of your visits. You also have the option to pay our self-pay cash rate and we’ll submit your claim to your insurance company to receive a partial or full reimbursement by your insurance, depending on your plan and its Out-of-Network deductible.

If your insurance is HMO or EPO, you do not have any out-of-network coverage and your option is to self-pay for our physical therapy services.

Verification Of Insurance Benefits

As a courtesy to our clients, we may offer to verify your insurance benefits through an online system called Availity. Sometimes this report does not offer extensive details of your in-network or out-of-network insurance benefits specifically pertaining to specialist services or a physical therapy services which we will share with you prior to your first visit. We highly encourage you to call your insurance directly with any questions you may have prior to your PT appointment to fully understand your benefits, to avoid any unexpected payment responsibility. We apply balance billing to all out-of-network

PLEASE NOTE: PAYMENT IS DUE PRIOR TO OR AT THE TIME OF SERVICE.

We will bill your insurance and track claims. Please keep us informed of any changes to your insurance plan.

For all out-of-network insurance plans, you are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your financial responsibility.

Choosing to pay for your services out-of-pocket will ultimately lead to needing fewer visits which is less costly, getting better outcomes which gets you to do what you love faster, and reduces spending on co-pays and unnecessary visits in the future. Whereas most PT clinics see clients 2-3x/wk for up to 21+ visits and can only treat you in a way that’s fully covered by insurance regardless of how effective it is, we usually see you 1-2x/wk (2-12 visits on average) and make it a point to ensure that you are staying on track outside of PT through giving you all of the necessary resources to learn how to manage your situation with confidence and autonomy. We also empower you with skills to heal yourself. The results are injury prevention in the future, and a better rehab process with recovery in a fraction of the time (depending on the injury).
